• Data through our booking request form and Assessment form to help potential clients communicate their nutrition needs while requesting a consultation/ assessment and to provide valuable nutrition advice and services to clients. Booking request form includes name and e-mail address, telephone number and type of services seeked. Assessment forms include your name, contact details, postal address, medical, dietary, nutritional and health history or concerns and its related information such as anthropometric data and information regarding your insurance coverage.

• Data related to any purchase of services / products / memberships /programs in person or through website including your contact details (name, address, email,phone number), purchase details (billing info) , credit card details

• Data through communication via online contact forms, web portal, client side mobile app, social media messages or posts, chat apps, texts or emails that you may send to us.

• Data related to any participation on

  • our website including (1) login details (name, user name, password, email address) to access a web-portal / membership area, (2) subscribtion to our newsletter, sign-up for free resources, opt-in for our offers, and to allow us to send you marketing or promotional materials, such as your name, email and marketing preferences;

PELTD only contacts individuals who specifically request that we do so or in the event that they have signed up to receive our messaging or have purchased one of our services.

Strict controls are maintained over access to the personal information you share with PELTD. Your information is safeguarded with two levels of password protection and accessible only by the Dietitian at PELTD.

Information is stored in a secured manner on a cloud server, list server by CRM systems and website management system that processes, stores and handles the data received through web forms and delivers PELTD content and messaging (as per instructions by PELTD) - collectively known as " third parties processors". Such Third Party processors may be located in another jurisdiction or outside Canada and may receive or otherwise have access to your Personal Information in its entirety or in part – depending on each of their particular roles and purposes in facilitating and enhancing our Services and business, and may only use it for such purposes and are bound by their respective privacy policies to maintain confidentiality and security.
